Due to the volume of applications we receive, although we try our best – we regret we cannot guarantee approval of every request. Please complete the following application if you are seeking support for an upcoming event, show or fundraiser.

– Please send in your application as soon as possible to allow us time to review your request

– Applicant will be notified if their donation request was approved/denied as soon as possible

– Please do not call the office to check the status of your application

– Past support of an organization is not a guarantee of future support

– If applicable, include 501(c)(3) letter from the IRS, Department of the Treasury
